We collect information from the Survey questions about you and others in your household, including contact information (such as your name, address, phone number, email address), demographic information (such as members, age, ethnicity, gender, and devices) and your TV viewership preferences.

If you voluntarily contact us to submit inquiries, request information, or give us feedback, we will ask for your contact information. We may ask you to provide other information in connection with your request, such as your preferred means of communication.

Information from your use of this Site. We may automatically receive certain information when you visit this Site. Please note that we may deny a request from an Authorized Agent if they do not submit proof that they have been authorized by you to act on your behalf.

We will not be able to respond to your request or provide you with personal information if we cannot verify your identity or the submitter’s authority to make the request and confirm the personal information relates to you. To verify your identity, we will seek to match any personal information that you provide when you (or an Authorized Agent) submit your request to personal information that we already maintain. This Survey and Site are not intended for minors under the age of 18 and we do not knowingly collect personal data about minors under the age of 18. If we find that we have collected personal data of a minor under 18 at this Site, we will delete it immediately upon learning about it.
